Leonard Jason's symptom questionnaire developed in 2010 for use in research studies and patient registries was the first to incorporate both frequency and severity of symptoms into an ME/CFS questionnaire.

The Symptom Questionnaire has been validated several times using a test-retest analysis, factor analysis, and as a diagnostic tool.

I was never given a test I had a very smart doc who concluded the blood cells EBV and tiredness not going away as CFS.I was believed back in 2003. Even Fibromyalgia they diagnosed. Have seen multiple pain docs who agree and 2 Rheumys. I wish I had this back then, it would have led to a faster diagnoses as I searched the internet and she searched for these symptoms.

This is much better than anything else I have seen. There are some things I feel it left out, so not perfect, but large improvement over any others I have seen. I wish my doctors had one more similar to this one than their mainstream ones, which do not address my salient difficult problems.
