Therapeutic actions:
Cerebrolysin is a protein-based agent that contains small-molecule biologically active neuropeptides. Neuropeptides penetrate through the blood-brain barrier and act directly on nervous cells.
Cerebrolysin possesses the unique organo-specific multimodal action on the brain manifested in metabolic regulation, functional neuromodulation, neuroprotective action, neurotrophic activity.
Cerebrolysin has been studied in thousands of patients in double-blind studies with consistently excellent results.
Indications:
Cerebrolysin is used to treat the following conditions:
  • Organic, metabolic and neurodegenerative disorders of the brain, especially senile dementia of type
  • Post-apoplectic complications
  • Craniocerebral trauma
  • Post operative trauma
  • Cerebral contusion
  • Concussion
  • Ischemic stroke
  • Chromic cerebrovascular pathology
  • Mental retardation
  • Disorders associated with attention deficiency in children
  • Senile dementia
  • Alzheimer's disease
  • Endogenous depression
Contraindications and Cautions:
Special care is indicated in cases of:
  • Allergic diathesis
  • Epileptic conditions and grand mal convulsions; Cerebrolysin treatment may result in an increase in the frequency of seizures
  • Although there are no indications that Cerebrolysin causes renal stress, the product should not be administered in the presence of existing severe renal insufficiency
Cerebrolysin should be used with caution in pregnancy(1 trimester and breastfeeding)
Side Effects:
There are possible side-effects associated with this medicine that can affect individuals in different ways. If a side effect is stated here, that does not necessarily mean the fact that all people using Cerebrosylin will experience it or any other.
Cerebrolysin is generally well tolerated. If injected too quickly it may cause a moderate heat sensation. In extremely rare cases hypersensitivity reactions manifest themselves as chills, headaches, or slight increases in body temperature of the patient. In no case to date has the undesirable effect persisted or proved threatening to the patient's health.

Interactions:
It is important to tell your doctor or pharmacist what medicines you are already taking, including those bought without a prescription and herbal medicines, before you start treatment with Cerebrolysin. Similarly, check with your doctor or pharmacist before taking any new medication while taking this one, to ensure that the combination is safe.
On the basis of Cerebrolysin's pharmacological profile, special attention should be given to possible additive effects when used in conjunction with anti-depressants or MAO-inhibitors. In such cases it is recommended that the dose of the anti-depressant is lowered.
Cerebrolysin should not be mixed with balanced amino acid solutions in an infusion.

Cerebrolysin is a peptide mixture isolated from pig brain. A neurotrophic peptidergic mixture produced by standardized enzymatic breakdown of lipid-free porcine brain proteins, cerebrolysin is composed of 25% low molecular weight peptides (<10K DA) and 75% free amino acids, based on free nitrogen content [1]. The mixture has relatively high concentrations of magnesium, potassium, phosphorus, and selenium [2], as well as other elements [3, 4]. While the drug has antioxidant properties, it is much less than trolox or vitamin E [5]. The active ingredient(s) in the mixture are not known. Two concentrates of the peptide fraction of cerebrolysin are being tested, one called EO21 and the other N-PEP-12 [6].

Abstract
Cerebrolysin is the only drug available for clinical use containing active fragments of some important neurotrophic factors obtained from purified porcine brain proteins, which has long been used for the treatment of dementia and stroke sequels. Cerebrolysin has growth factor-like activities and promotes neuronal survival and sprouting, however, its molecular mechanism still needs to be determined. It has been shown that Cerebrolysin may interact with proteolytic pathways linked to apoptosis. Administration of Cerebrolysin significantly reduces the number of apoptotic neurons after glutamate exposure. Furthermore, it has been reported that Cerebrolysin inhibits free radicals formation and lipid peroxidation. In vitro we evaluated the protective effects of Cerebrolysin towards spontaneous and induced apoptotic death in cells from healthy individuals. Peripheral blood lymphocytes (PBLs) from 10 individuals were used as cell model; 2-deoxy-D-ribose (dRib), a highly reducing sugar, was used as paradigm pro-apoptotic stimulus. Apoptosis was analysed using flow cytometry and fluorescence microscopy. Our results showed that Cerebrolysin significantly reduced the number of apoptotic PBLs after dRib treatment, although it had no significative effects on cells cultured in standard conditions. Our work showed a protective effect of Cerebrolysin on oxidative stress-induced apoptosis and suggested that PBLs can be used as an easy obtainable and handy cell model to verify Cerebrolysin effects in neurodegenerative pathologies.
