In a first for it's chronic fatigue syndrome research program, NIH has begun a creative grant expansion program to jump start ME/CFS research in the short term. Scientists with current NIH grants can apply for supplemental funds to expand existing studies. If this initiative is followed by RFAs – as NIH’s revised response to CFSAC suggests is forthcoming – the combination could immediately boost research with some additional funds, possibly attract new investigators through incentives to add ME/CFS research to current studies, and build the capacity for expanding the medium term research program at NIH.
Suzanne Vernon of the Bateman-Horne Center noted that the grant could help fulfill a vital need by bringing young, new researchers in the form of post-graduate students into the field.
Five institutes including the immune and neurology institutes (NIAID, NINDS) have agreed to allow past award winners to apply to bump up past grant awards by a hundred grand or more if they want to add new assays, statistical analyses or new disease cohorts to their studies.

[/fright] Since grants typically run about $2-500,000 a year this award could add a significant amount of funding to an already approved grant.
The "areas of interest" are all good: they include central nervous system, blood, spinal fluid, etc, biomarkers, autonomic abnormalities (yah!), ME/CFS pathogenesis, immune modeling, the microbiome and others.
Bonus Package: Adding ME/CFS Patients to Other Studies
The funding for the first part of the announcement is limited by the 10 or 15 grants on ME/CFS being funded right. The second part of the announcement that allows for other researchers to add ME/CFS cohorts to their projects has no such limitations. This part of the grant offers the opportunity to get established researchers in other diseases involved in ME/CFS.

[/fright]This new program should be a hit. Because the NIH wants to add funding and they've already signed off on these grants most requests will probably be approved. It's hard to imagine, for instance, that Ian Lipkin and Mady Hornig would not apply for increased funding for their expensive microbiome study and not get it. The same is true for Lenny Jason's big infectious mononucleosis study; he wants more testing for that and he should be able to get it.
This effort could add $1,000,000 or more to ME/CFS research funding.

The Working Group appears to be leaving no stone unturned thus far in their efforts to get ME/CFS funding moving. The major component of their effort - RFA's and such - will require approval from the Institutes. We won''t know their fate until May. In the meantime this effort is a good sign that this Working Group is indeed different from the WG's we've seen in the past.

- Characterization of CNS-related biomarkers between new onset and established ME/CFS patients and appropriate comparison groups
- Identification of biomarkers in cerebrospinal fluid, blood, urine, etc. that can identify physiologically relevant subgroups of ME/CFS
- Studies elucidating autonomic abnormalities seen in the central or peripheral nervous system of patients with ME/CFS
- Studies to provide a better understanding of ME/CFS, prevalence, pathogenesis, and pathophysiology
- Identification of potential triggers or modifiers of immune responses or immune cell metabolism that contribute to ME/CFS
- Application of current methodologies (e.g., immune phenotyping) or computational modeling to better characterize human immune responses triggered by ME/CFS
- Elucidation of etiology of ME/CFS and physiological determinants involved in ME/CFS manifestations including but not limited to microbiome-related studies.

- A Case Western researcher, for instance, studying mitochondrial bioenergetics in cancer-related fatigue could conceivably add an ME/CFS cohort to his study.
- Steven Shutzer - who has done work in ME/CFS - is looking for protein biomarkers that show up early in multiple sclerosis. (NINDS.)
- Lea Ann Matura is studying the effects of sympathetic nervous system activation and inflammation on sleep, breathing problems and fatigue in women with pulmonary arterial hypertension. (NINR)
- Bradely E. Aouizerat is studying epigenetic profiles in patients with cancer (NINR)
- LEOREY Saligan is investigating molecular genetics and therapeutics for fatigue in cancer
- Jeffrey Scott Wefel is examining biomarkers using MRI's for cognitive problems in cancer fatigue (NINR)
- Canhua Xiao, a young researcher mentored by Andrew Miller is examining the role of pro and anti-inflammatory signaling in cancer fatigue.
