What clinics offer Ampligen?

Does anybody know the supposed 4 clinics in the states offering Ampligen to severely ill patients?

Dr. Lapp in the Hunter-Hopkins center is the only place I know of.

Dr. Peterson in Incline Village does.

I believe that Dr. Bateman in Salt Lake City must as well - she testified on Ampligen's effects at the FDA hearing.

That's three...Who is the fourth?

Klimas seems to be open to everything reasonable....Kogelnik? Levine?
