On-line screening questionnaire for chronic fatigue and pain:

The purpose of this screening questionnaire is to see if you meet the criteria for taking part in our research study. The purpose of this blood draw study is to learn more about chronic conditions such as chronic fatigue syndrome (CFS), myalgic encephalomyelitis (ME), systemic exertion intolerance disease (SEID), and fibromyalgia (FM). This study is not testing a new treatment, but is designed to develop new diagnostic tests. The study is funded by a large grant from the National Institutes of Health (NIH).
